New Patient forms
Please arrive 10-15 minutes prior to your scheduled appointment time to fill out new patient intake forms. This only needs to be done on your first visit.
What should I expect on the first visit?
Your first visit will include a consultation and comprehensive history and examination. The doctor will go over your health history and determine the proper treatment specific to your needs. You will be given a full explanation of the examination findings and the treatment plan prior to treatment.
Do you accept insurance?
Yes, most insurance plans are accepted. It is recommended that you provide your insurance information prior to your first visit to determine insurance eligibility. Some insurance verifications may not be processed immediately and may take up to a few business days.
Where are you located?
The office is located at the corner of 7th & Wilshire, conveniently close to downtown Santa Monica and 3rd Street Promenade. There is a paid parking lot directly adjacent to the building, or metered parking is available along 7th Street. The office is located on the 3rd floor of the historic, art-deco professional building. Please enter the building and take the elevator to the third floor, the office is in suite #314.

Tuesday: 8am - 8pm
Wednesday: 2pm - 7pm
Thursday: 8am - 8pm
Friday: 8am - 11am
Saturday: 8am - 1pm
